Wills and Estate Planning Intake Form:

BOOKING STEPS & IMPORTANT INFORMATION

  1. Complete the intake form to the right (if on a computer) or below (if on a mobile.

  2. Proceed to the booking page to arrange a tentative appointment in person or by Microsoft Teams (please select your preferred choice from the options).

  3. Check your email inbox. You will have a confirmation email with our costs schedule, estate planning questionnaire for completion and other helpful information to prepare for your appointment. This will ensure, subject to the below, everything goes ahead as smoothly as possible.

  4. Complete your estate planning questionnaire. This is a PDF fillable form. Alternatively, you can print and complete in pen. Send your estate planning questionnaire and any other relevant documents by email to admin@ellerylegal.com.au prior to your appointment or bring a printed copy with you.

Please note, however, that as we cannot provide legal advice in matters that are not within our areas of expertise and/or where we have a conflict of interest, once we have your completed intake form we will need to consider if we cannot assist with your appointment for any reason.

If we cannot, we will unfortunately need to cancel your appointment. You will receive notice of cancellation via email, in which case we sincerely apologise for any inconvenience and appreciate your understanding.
